Please fill out this form and email to the NORCAL General
Manager at least 7 days before playing any game against a
team that is not a member of NORCAL, either locally or out
of town.

A $100 fee will be assessed if the team does not apply on time. Also 
note that if a team is going to leave the state of California, a state 
travel permit must be issued by the CAHA member services director.
